REQUIRED FORMS

You will be emailed and required to complete the forms linked below before arriving for the weekend, and bring along with you a picture ID. THESE FORMS MUST BE COMPLETED THROUGH THE ADOBE SIGN EMAIL YOU WILL RECEIVE AFTER REGISTERING.

Once you print and fill out the forms, you can use an iPhone Notes App or alternative app to scan the filled-out documents and email them back to this email address with the title "PCW Forms (First and Last Name)". You may leave Host Name Blank."

PACKING LIST 

Don't forget to bring these items!

• A valid photo ID (school ID is ok)
• Change of clothes for 2 nights and 3 days!
  • You are required to wear the provided PolyCultural Weekend t-shirt for the duration of the event, but extra clothes are recommended.
• Jacket/sweatshirt
• Umbrella, just in case 
• Comfortable shoes
  • You will be walking a lot and the campus has some fun terrain!
• Pajamas
• Towel and shower toiletries
  • Many of the residence halls you may be staying in with your host have communal showers and so shower shoes/flip-flops are also recommended
• Toothbrush, toothpaste, mouthwash, floss
• Sleeping bag and pillow, BEDDING IS NOT PROVIDED
• Any prescription medications
• Allergy medication
  • There is a lot of pollen in the air here. If you have seasonal allergies/pollen allergies, it is recommended you bring some over-the-counter allergy medication such as Zyrtec, Claritin, generic equivalent, etc.
